When you already have a gooseneck hitch in your 2002 Chevy 3500, you unfortunately won't be able to add base rails as they will interfere with the gooseneck hitch mounts. There are adapters for some gooseneck systems that allow you to use more standard hitch options. The B and W Companion hitch part # BWRVK3500 can only fit trucks that have a B and W Turnoverball gooseneck hitch installed in them like the part # BWGNRK1384 that fits the 2018 Ram 2500. There isn't a way to fit this hitch to any other brand of gooseneck.

The Hi-Rise 5th wheel hitch # 9480-5W comes with 2 shanks that would fit down inside the gooseneck ball hole. There is a square one for B&W gooseneck hitches and a round one for Hide-A-Goose hitches by Draw-Tite or Reese or under-bed models from Bulldog. I may have another option for you, but the Curt 4" Offset Ball for Double-Lock Gooseneck Hitch # C602 you mentioned will not work in the factory gooseneck in your 2020 Ram 3500. The factory gooseneck uses ball bearings to keep the ball in place, and due to that design there is nothing to keep the ball from rotating so an offset ball is not available.
